Pharmacy Technician – Medication reconciliation

Responsibilities

The Medication Reconciliation Technician prepares detailed home medication lists utilizing a variety of tools. Duties include review of medical records for previous medication history, interview with patients and/or family members, communication with physician offices and/or pharmacies, and entry of medication information into the electronic medical record. Position may also assist pharmacy with day-to-day dispensing activities and record maintenance when needed. Works under the supervision of a licensed pharmacist, practicing under the scope of State and Federal laws and statutes.
Qualifications

  • Knowledge of pharmaceutical generic and trade names and their dosage forms, and the ability to cross-reference common drugs from memory.
  • Ability to accurately perform pharmaceutical calculations.
  • Knowledge of medical terminology and pharmaceutical abbreviations.
  • Ability to work independently and follow-through on assignments with minimal supervision.
  • Telephone and verbal skills required to communicate clearly and effectively with pharmacists, nurses, physicians and other allied health professionals.
  • Requires intense and consistent concentration with attention to detail, and requires speed and accuracy.
  • Requires normal range of hearing and vision in order to communicate and perform effectively.
  • While performing the duties of this job, the employee is frequently required to stand, sit, and walk.
  • Requires ability to lift 25 lbs and push 50 lbs.
  • CPR certification will be required within 90 days of hiring.
  • CA Pharmacy Technician License – required.
  • Inpatient pharmacy technician experience – highly preferred.
