Responsibilities
The Medication Reconciliation Technician prepares detailed home medication lists utilizing a variety of tools. Duties include review of medical records for previous medication history, interview with patients and/or family members, communication with physician offices and/or pharmacies, and entry of medication information into the electronic medical record. Position may also assist pharmacy with day-to-day dispensing activities and record maintenance when needed. Works under the supervision of a licensed pharmacist, practicing under the scope of State and Federal laws and statutes.
Qualifications

Knowledge of pharmaceutical generic and trade names and their dosage forms, and the ability to cross-reference common drugs from memory.

Ability to accurately perform pharmaceutical calculations.

Knowledge of medical terminology and pharmaceutical abbreviations.

Ability to work independently and follow-through on assignments with minimal supervision.

Telephone and verbal skills required to communicate clearly and effectively with pharmacists, nurses, physicians and other allied health professionals.

Requires intense and consistent concentration with attention to detail, and requires speed and accuracy.

Requires normal range of hearing and vision in order to communicate and perform effectively.

While performing the duties of this job, the employee is frequently required to stand, sit, and walk.

Requires ability to lift 25 lbs and push 50 lbs.

CPR certification will be required within 90 days of hiring.

CA Pharmacy Technician License – required.

Inpatient pharmacy technician experience – highly preferred.
